New Patient Forms
Thank you for turning to us for your chiropractic care. We are committed to making your initial visit and subsequent care a comfortable and satisfying experience. At Maples Chiropractic we see patients by appointment and on an urgent or emergency basis. To schedule an appointment at our office, call (317) 270-9500. For your convenience, please click on the new patient forms link below and fill out new patient paperwork. This will save you time at your initial visit in our office.
If you must cancel an appointment, please call at least 24 hours in advance. Please bring appropriate insurance information, including your ID card, to your first appointment. We file all insurance forms, when appropriate. We participate in most all health plans. If you have specific billing or insurance questions, you may call our office ahead of time.
If your primary care physician refers you to Maples Chiropractic, we will stay in touch with him or her while you are under our care.
If you have an urgent need (not life-threatening), office phones are transferred during non-business hours with someone on call 24 hours-a-day, seven days-a-week. If you are facing a life-threatening emergency, go to the nearest hospital emergency room.
